clindamycine po 600mg 3dd 2 to 4 weeks

flucloxacilline po 1000mg 5dd 2 to 4 weeks
Medication:

flucloxacilline iv 1000mg 6dd 4 to 6 weeks

or

flucloxacilline iv loading dose 1g, 6g continuous per 24 hours 4 to 6 weeks
Medication:

cefazoline iv 1g 3dd 2 weeks

followed by

clindamycine po 600mg 3dd 2 to 4 weeks

General comments

Flucloxacilline per os hooggedoseerd alleen indien goed verdragen en met spiegels bewezen goede resorptie

flucloxacilline iv 1000mg 6dd  of flucloxacilline iv oplaaddosis 1000 mg, 6000 mg continue per 24 uur
