Comments:

Bij patiënten met morbide obesitas kan de AUC en Cmax van gentamicine gewijzigd zijn.
Doseeradvies bij (morbide) obese patiënten met een geschatte nierfunctie > 60 ml/min:

Gewicht tot 100 kg: gebruikelijke dosering (5 mg/kg TBW).
100 - 120 kg: 480 mg 1x per dag.
120 - 140 kg: 560 mg 1x per dag.
140 - 160 kg: 600 mg 1x per dag.
160 - 180 kg: 680 mg 1x per dag.
180 - 200 kg: 760 mg 1x per dag.
200 - 220 kg: 800 mg 1x per dag.

Met deze doseringen is een vergelijkbare blootstelling te verwachten als met non-obesen die 5 mg/kg TBW krijgen. Er wordt geadviseerd om de dosering na 1 gift te individualiseren op basis van Therapeutic Drug Monitoring.

Renal function-replacement therapy

CAPD: Normale aanvangsdosis, vervolgens interval verlengen op geleide van de spiegel.

Hemo dialysis: Normale aanvangsdosis, vervolgens interval verlengen op geleide van de spiegel. Op dialysedagen: bij toedienen voor dialyse normale dosis; bij toedienen na de dialyse de helft tot een derde van de dosis.

CAV / VVHD: Normale aanvangsdosis, vervolgens interval verlengen op geleide van de spiegel.
